2. Click here for our Mail Service Agreement. This is a Acrobat PDF file. Please print, complete and send to our office along with the Postal Form 1583. If you are unable to open please contact our office.

3. The third form is the Postal Form 1583. This is required by the US Postal Service and it authorizes us to handle your mail. Please complete, sign, have notarized and return the original to our office. Click on the link below and it will take you to the USPS web site where you can obtain the form. The URL is http://usps.com/forms/_pdf/ps1583.pdf.
